Strathmore Day Opportunities

Strathmore Day Opportunities provides a Day Opportunities service for older people, including those with dementia. We support individuals to lead more active and interesting lives in their community.

Aims and objectives

  • To provide a quality, person-centered, flexible and evidence based service that promotes and supports individual strengths and levels of independence.
  • To offer a wide range of activities and opportunities to promote and maintain social, physical and mental health well-being.
  • To enable clients with dementia to continue living within their own homes for as long as possible.
  • To fully involve and support service users and carers in all aspects of our service.
  • To deliver our service within a friendly, safe and welcoming environment.

Quality

Our service is underpinned by the Health and Social Care Standards which set out what you can expect, as a minimum, from us, and by the Scottish Social Service Council's Codes of practice which set the standards of professional conduct and practice required of social service workers. Our service is registered, regulated and inspected by Care Inspectorate, Compass House, 11 Riverside Drive, Dundee, DD1 4NY, Tel. 0345 600 9527. Participation

In order to continually improve our service we encourage the views and suggestions of clients, carers and others. We use a range of methods to gather these including advocacy services to further support our clients and carers in this area. If you would like more details of how you can become involved, please speak to a member of staff and ask for a copy of our Communication, Engagement, and Consultation strategy.

The Centre

We are open Monday to Friday from 8.45 am to 4.00 pm. We offer day opportunities, outreach support and carers support.

Some of our services are charged for. You may be required to pay in full or in part for the day opportunities you receive. The amount which is chargeable is based on the number of times each week which you attend the day opportunities. Costs will be discussed with you individually and you will be offered a financial assessment and advice on income maximisation. Charges for day opportunities include the costs of internal transport, but meals must be bought individually on the day.

Staffing

The service has a number of staff who are supported by volunteers, modern apprentices and students. The Day Opportunities Co-Ordinator is also at the building Tuesday to Friday and depute manager Monday to Thursday.

Each service users is allocated a key worker. Your key worker will work with you to develop, and regularly review, your Person centered plan to ensure that the service is meeting your expectations, goals and wishes.

Accessing the Service

You can access Strathmore Day Opportunities by a referral to:

Health and Social Care Partnership, Early Intervention and Prevention Team - AccessTeam@pkc.gov.uk or on 0345 30 111 20.

Following the social work assessment, you will be contacted by staff at Strathmore Day Opportunities to arrange your day opportunities.

Complaints

We strive to provide the best possible service at all times but sometimes things do go wrong. If you have a complaint about any part of the service you receive from us, please contact the Day Opportunities Co-ordinator or Key worker.
