Statutory forms and related guidance to be used as a requirement of The Burial (Applications and Register) (Scotland) Regulations 2024.
These seven statutory application forms (which include guidance notes at the end of each form) have been made under The Burial (Applications and Register) (Scotland) Regulations 2024.
These statutory forms must be used to apply for a burial at any burial ground in Scotland. The forms can be completed electronically or printed and completed by hand.
If you are arranging a funeral, and you are using a funeral director they will be able to advise you about which form you will need to complete.
If you are not using the services of a funeral director and are making the arrangements yourself, you should speak to your chosen burial authority directly to make the necessary arrangements for the burial.

- Application for burial of the remains of a pregnancy loss in a burial ground (by individual) (PDF, 483 KB)
- Application for burial of the remains of a pregnancy loss by health authority or body (shared or individual) (PDF, 352 KB)
- Application for burial of the remains of an adult or child by a local authority where they are making arrangements under section 87 of the 2016 Act (PDF, 436 KB)
- Application for burial in a burial ground of body or whole body/body parts donated for anatomical examination on or after the date of commencement of the Anatomy Act 1984 (PDF, 443 KB)
- Application for burial in a burial ground of body parts donated in Scotland and in possession of a person licensed under section 3(2) of the Anatomy Act 1984 before the commencement of the Anatomy Act 1984 (as amended by the Human Tissue (Scotland) Act 2006 (14 February 1988)) or where the date of death is not known or is before 14 February 1988 (PDF, 361 KB)
